Newborn Assessment


The Newborn Assessment checklist incorporates 32 assessment criteria beginning at reviewing patient charts all the way through a complete physical assessment of the newborn. The checklist can come with a designated amount of points assigned to each assessment criteria (totaling 100), or the checklist can be available with blank columns to be populated by each user independently. For clinical use, the criteria assessments are not assigned points, and the checklist is used as a tool to comment on the newborn’s physical assessment.


An assessment criteria will include a section header (e.g., “Nose”), with instructions to assess various sub-headings (e.g., “Placement,” “Patency of nares,” “Nasal Flaring,” and “Drainage”). In addition, each assessment criteria includes normal limit ranges and further instructions for assessing each sub-heading (e.g., “Occlude one nares at a time while keeping infants mouth closed”).


The assessment also includes criteria pertaining to pre- and post-assessment tasks such as “Compare infant’s identification band name with the parent band name.” It can thus be used as a complete training tool for prospective nurses, or as a comprehensive clinical checklist.
